You are asked to pro­vi­de your details (name, e‑mail address, time avai­la­bi­li­ty) in the ques­ti­onn­aire below. All infor­ma­ti­on is vol­un­ta­ry. The ques­ti­onn­aire is used to select work­shop participants. 

All data will be trea­ted con­fi­den­ti­al­ly and will not be pas­sed on to third par­ties. The data will be dele­ted at the latest after the selec­tion of the candidates.

The par­ti­ci­pants are peop­le who are inte­res­ted in the deve­lo­p­ment of an app to sup­port their dise­a­se. For this rea­son, they are asked to pro­vi­de their details (name, email address, time avai­la­bi­li­ty) in the ques­ti­onn­aire below. 

Data to be collected/stored 

We will collect the fol­lowing per­so­nal and health data: 

- Name 

- E‑mail address 

- Lan­guage skills 

- Acces­si­bi­li­ty 

- Exis­ting illness 

- Gene­ral questions 

This data is collec­ted by means of a pre­de­fi­ned ques­ti­onn­aire. The choice of ans­wers is optional. 

Fur­ther­mo­re, usa­ge data is collec­ted when the ques­ti­onn­aire is used.

The­re is an order data pro­ces­sing agree­ment with Type­form 

After com­ple­ti­on of the sur­vey, the data will be dele­ted from Type­form and stored local­ly by the respon­si­ble par­ty only. The data will only be made avail­ab­le to a limi­ted group of peop­le and not to third parties. 

Dura­ti­on of sto­rage 

The data will only be stored for as long as is necessa­ry for the selec­tion of participants. 

Legal basis 

The legal basis for sto­rage is con­sent in accordance with Art. 6 Para. 1 lit. A GDPR. 


You have the right to revo­ke your con­sent at any time. Sin­ce the revo­ca­ti­on app­lies exclu­si­ve­ly to the future, the law­ful­ness of the pro­ces­sing car­ri­ed out on the basis of the con­sent until the revo­ca­ti­on is not affec­ted. The revo­ca­ti­on must be addres­sed to the per­son respon­si­ble. After rece­i­pt of the revo­ca­ti­on, the data pro­vi­ded will be deleted.
