You are asked to provide your details (name, e‑mail address, time availability) in the questionnaire below. All information is voluntary. The questionnaire is used to select workshop participants.
All data will be treated confidentially and will not be passed on to third parties. The data will be deleted at the latest after the selection of the candidates.

Participants
The participants are people who are interested in the development of an app to support their disease. For this reason, they are asked to provide their details (name, email address, time availability) in the questionnaire below.
Data to be collected/stored
We will collect the following personal and health data:
- Name
- E‑mail address
- Language skills
- Accessibility
- Existing illness
- General questions
This data is collected by means of a predefined questionnaire. The choice of answers is optional.
Furthermore, usage data is collected when the questionnaire is used.
The questionnaire is created with the help of typeform.com. The data protection regulations apply:

After completion of the survey, the data will be deleted from Typeform and stored locally by the responsible party only. The data will only be made available to a limited group of people and not to third parties.
Duration of storage
The data will only be stored for as long as is necessary for the selection of participants.
Legal basis
The legal basis for storage is consent in accordance with Art. 6 Para. 1 lit. A GDPR.
Rights
You have the right to revoke your consent at any time. Since the revocation applies exclusively to the future, the lawfulness of the processing carried out on the basis of the consent until the revocation is not affected. The revocation must be addressed to the person responsible. After receipt of the revocation, the data provided will be deleted.
